If you have seen another doctor, bring copies of these records or ask them to send a report to our office.  Have your medical records faxed attention to our Referral Coordinator at (706)790-1240.

On the day of your visit:

• Put all of your medicines into a bag including over-the- counter-medicines and herbal medicines and bring them with you to the visit.
• Bring in a list of questions and concerns that you want to discuss with the provider during the visit.
• Bring any self-monitoring tools (BP or Glucose logs) to the visit.
•  Bring all of your insurance cards and a picture ID.
•  If you need help ask a family member or friend to come with you to the visit.
• Tell the provider of any referrals you may need.

Yes. We have Sliding Fee Scale discounted program (SFSD).

Please provide one of the following within 30 days of you signing the application;

•  Current Pay checks for a full month
•  Income tax return for previous year
•  Retirement incomes indicated by bank statement or check
•  Social Security benefit
•  Alimony
•  Child Support
•  Disability statements
•  Pension
•  If no income or do not have a job, a wage form must be obtained from the Georgia Department of Labor

• Provide us with as much information about you and your family’s medical history as you can.
• RELAX!!! Ask questions. Take notes. Let your doctor know when you don’t understand. Remember your doctor wants the very best for you.

We offer same-day and work-in appointments daily for routine and non-emergent acute care issues between the hours of 8am and 3pm. Call any of our locations to schedule an appointment.

If it is a medical emergency, please dial 911. For non-emergency care after hours, please call the office at (877) 205-5006 . You will be directed to our answering service attendant who will connect you with our on-call provider so that you can get help with your medical problem.

Our In-House Pharmacies and Dispensaries are available to fill your prescriptions as soon as you finish your office visit. We can also have your medicines delivered directly to your home.

When you log on to the patient portal you will be able to view:

• Your health information
• See your recent lab or test results
•  Request medication refills
•  Request an appointment
•  Send a secure messages to your provider
