Our health center is participating in a patient survey that will provide important information about patient experiences with telehealth. We want to hear from you about how your telehealth office visit made a difference for you.
The survey takes less than 5 minutes to complete and no personal information is needed. The survey results will help health centers nationwide to promote healthcare equity and improve the patient experience.
Patients who complete the survey and enter their email address (optional) will be entered into a lottery for one of ten $50 gift cards.
