Before your visit, make sure to: Gather and bring your medical history, including any medications you are taking. Make a list of symptoms or questions you want to discuss. Bring your insurance information, ID, and any required paperwork. Arrive a few minutes early to complete any additional forms.
For your first visit, please bring: A valid photo ID. Insurance card (if insured). Co-payment. A list of medications and allergies. Any recent medical records or test results, if available.
Yes, we provide healthcare services to all patients, regardless of insurance status. We offer a sliding fee scale based on income and family size, so uninsured patients can still receive care at an affordable rate.
If you are uninsured, please bring:
1. A valid photo ID.
2. Provide proof of income within 30 days of signing the application: Current paychecks for a full month, Income tax return for previous year, Retirement incomes indicated by bank statement or check, Social Security benefit, Alimony, Child Support, Disability statements, Pension.
If no income or do not have a job, a wage form must be obtained from the Department of Labor and any additional documents related to financial assistance programs, if applicable. This will help us determine if you qualify for discounted services.
